Open Enrollment packets were mailed out to each firefighter’s home this week.
Please visit www.local157.org to view/print copies of the plan summaries and additional information from carriers. If you would like a printed copy please come to the Union during the Open Enrollment dates listed above to get your City of Oklahoma City booklet or VEBA Trust Benefits Booklet.
If you do not have any questions or changes for the new plan year, it’s not necessary for you to attend Open Enrollment however you will need to enroll by mail.
You can make benefit plan changes or confirm your 2010 election by signing the 2010 Open Enrollment Form and return it in the envelope provided in your packet no later than
November 19, 2009.
IMPORTANT REMINDER: (Active Employees only) Flexible Spending Accounts - If you currently have or want to enroll in a Flexible Spending Account, you are required to complete
the City of Oklahoma City Personal Enrollment Form enclosed in your packet, sign and return in the envelope or bring to Open Enrollment. You are required to make an election each year or
your Flexible Spending Accounts will not be deducted for 2010. For more information on your flexible spending accounts or other City benefits, please refer to the 2010 City of Oklahoma City
Firefighters Reference Guide at www.local157.org.
IMPORTANT: Please review the enclosed plan comparison. Office Visit Copays and Prescription Copays for all plans have increased this year and are listed on the
comparison and within the Plan Summary available on the Local 157 website.
All benefit changes become effective January 1, 2010.
Dependents - review the family members you have covered. During the annual enrollment period, you are verifying that your dependents meet the VEBA Trust’s benefit eligibility
requirements.
By taking no enrollment action you will remain enrolled in the same benefit plans and premiums will automatically adjust to the new rates with the
exception of Medical or Dependent Care Flexible Spending Accounts (FSA). These require re-enrollments each year with the City of Oklahoma City.
